This document is a large 100 x 150 cm poster describing the steps required to institute and operate a mishap investigation and prevention quality assurance program. It is posted for downloading and display in classrooms, or at seminars and conferences. Anyone who wishes to download and display it has our permission to do so, without fees, provided all the text and graphics, including the copyright notice, are shown on the display. Other uses are not authorized without prior permission of the authors.
This poster was first displayed at a conference in Albuquerque NM in 1994.
To view the files, your browser should have both a vertical and horizontal scroll bar. To use it for display purposes, you will need the following information. The poster files contain a set of 30 pages for attachment onto a rigid backing panel. They are laid out in a matrix 5 pages vertically by 6 pages horizontally. The 30 pages are contained in 6 files, shown as Part a through Part f. The pages are laid out for printing in landscape print orientation. as shown.
The files are posted as GIF files. They can be downloaded and converted to fit your graphics applications. The posters contain red headings and green numbers. All the other text and illustrations are black. To convert the .gif files for the Mac or PC, use a graphics file converter such as one of those that can be downloaded from internet web sites.
