About a year ago, a paper was posted at this site comparing reports of 3 different fire investigations of the same incident. THE INVESTIGATIONS PRODUCED THREE DIFFERENT RESULTS. Recently, a paper reporting the results of a reinvestigation of an aircraft accident was posted at this site, accompanied by a link to an earlier report of the same accident. The INVESTIGATIONS PRODUCED TWO DIFFERENT DESCRIPTIONS AND EXPLANATIONS OF WHAT HAPPENED. Another example that has come to my attention will be posted shortly.
These differences raise two obvious questions.
A recent paper suggests another question:
One way to stimulate development of improved investigation performance may be to make publicly available similar examples of accidents where two or more investigations produced different outcomes. In keeping with the exploratory nature of this site, this section of theIPRR will be start carrying Petitions for Consideration of Probably Cause to the National Transportation Safety Board in the USA, the original report, and the Board's responses to such petitions. The intent of this undertaking is to learn how everyone might do better investigations by comparing investigations, and not as attacks on any of the parties involved.
A FOIA request for a list of Petitions received by the NTSB for the last 5 years was made in July 1999. To date, the data requested has not been made available. When it is, additional postings will follow.
